Extremely Low SHGB, Low Testosterone, High Free T, No libido, only 19.
Hey guys, just got back from my doctor in Long Island.
Basically, I've had a very low libido my whole life. This past summer I finally went to a urologist who tested my testosterone: 268, put me on cpyionate shots for 6 weeks. I came off because I noticed absolutely no improvements whatsoever. Fast forward a few months, saw Dr. Barrett, had a slew of labs done, now just got them back today. Here is the important stuff: (I have been off everything for a few months, not taking any medications) Also BP taken today was 140/80 and I weigh 220lbs, 5'9, although I am not fat, just have absolutely no muscle, skinny frame, but a belly I guess. Labs done by Sunrise Medical Laboratories.

HDL: 41(over 40)
LDL: 150(under 130)
Alkaline Phosphatase: 54(60-270)
Testosterone: 119(160-726)

%Free Testosterone: 3.5 High(1.50-3.2)
Estradiol: 41: (0-56) How can zero be an acceptable value???????
Progesterone: 0.4(0.0-.09)

T3 Total: 1.53(0.80-2.00)
T4: 7.1(4.5-11.7)

Vitamin D: 33(30-100) This does not make sense to me, I've been supplementing with 10,000 iu D-3 everyday for the past year, why isn't it higher?

Estriol Serum: 0.02(00.00-0.16) What is this and how does it differ from Estradiol?
All in all I am satisfied with this doctor, at least compared to the vast majority of others. IGF-1 is not listed here but I believe they ordered it, it is just taking longer than expected. He did not test free T-4 which confuses me. He could not explain why my SHGB is so low. He said it is good to have low SHGB as it frees more testosterone but I believe mine is way too low, thus not allowing testosterone to circulate and be delivered to receptors. This is my theory as to why I did not feel better on the shots. My research told me that low SHGB is a symptom of Hypothyroid, which is what I assumed I had, with my TSH being so high, but now my FT-3 contradicts that. It is over the range, which would suggest hyperthyroid. So I have no idea what is going on here.
He has prescribed a regimen of HCG and Nolvadex to be taken for the next 6 weeks. I will take the HCG for the first 2, then discontinue and start Nolva. He wants a blood test 1 week after cessation but I will wait at least 3, maybe 4 to get a true value. If this does not work, then I will go on hormone replacement therapy with a lose dose HCG and possibly anti-estrogen. My concern though, is that my libido will not improve on this without something else in the equation because of my low SHGB. Any thoughts?
Edit: also, why is my alkaline phosphatise low on a consistent basis? I've had a number of blood test and this number always hovers around 54. And how can I have such low total testosterone while having high free%testosterone?

Dude sometime the medical profession fails at its worse. I see your 5'9" 220 lbs that with low test, low libido. Are your snoring loud enough it bothers people if they are in the same room as you, do you wake up occassionally with a slight headache, do you wake up in the morning just as tired as when you went to bed. I know you blood pressure is up and these signs are indicative that you have sleep apnea ( stopping breathing in you sleep) When you don't get proper sleep and REM sleep you don't produce testosterone. Its a serious problem bro, I just put up a thread the other day about a kid that is 21yrs old and his test is 140, guess what was causing it sleep apnea. Evaluate yourself and research it a little and educate yourself and maybe if this is the problem you can teach your doc a thing or 2, and tell him you would like a sleep study done then go from there. JMHO
It sounds like you have to much estrogen vs testosterone. high estrogen will cause your T3 to be elevated. Worst case scenario you might have tumor on you testes. I understand your concern as your doctor is just treating your symptoms and you need a cure. I would stick with the prescribed regiment as it will greatly improve your life as you keep looking for the root cause. What you describe also happens to vegetarians who use soy as their only source of protein. As far as the your thyroid issues, the easiest way to diagnose hypothyroidism is to check your finger nails for bumps and ridges. Your low alkaline phosphatase is also caused by high estrogen. It's a long shot but you may have been born with both testes and ovaries. If after treatment your test levels do not improve then your might want to look into some of the issues I have mentioned.
You could have a pituitary tumor.. There's usually a cause for these types of things.. I'm sure your doctor has already explained that part to you.. If there are NO drugs, meds, allergies etc (it could be as simple as your allergic to a food you don't even know it, it just makes you tired but in reality it suppresses bodily functions) and you were healthy before then there has to be something causing it.. I would test and test and test over and over until I found the answer because it could end up being something serious down the road. 19 year old with low t and high thyroid production just sounds like your body is fighting back against something. If the thyroid production is high then in theory you should have higher test not low.. GH handles a lot of your other hormone levels. I would nag my doctors is all I'm getting at, that's my 2 cents
No, no steroids, except for the testosterone my previous urologist was giving, I have been off that for a few months though. What angers me is that I just saw a new endo today for a second opinion and she basically told me my test levels are fine. You have to look at not the total test but the free testosterone, as that is all that matters, and in that regard, according to her, I am fine. Obviously I am not seeing this person ever again. My reason for seeing her in the first place was for a diagnosis on my thyroid. I feel that my thyroid is out of whack and I just want answers as to why my TSH is high, suggesting hypothyroidism, while my FT-3 is over the range high, suggesting hyperthyroidism. This is a contradiction and I have no idea why my levels are like that. She could not answer this and basically said my thyroid is fine as well, it does not matter if levels are out of range, everyone is different.
I am just worried that even if I get my test levels back naturally, or if I have to get on hormone replacement therapy, my libido will never improve because of my low SHGB.
Edit: No other drugs either. I used to be a hardcore long distance runner, around the time I should have been going through puberty. That could be a potential reason for my messed up HPTA.
Your test is clearly very low. Finish your labs and follow doctors regimine. It looks like trt for you. On trt there will be labs after several Weeks to check levels and adjust if needed. Go thru the hurdles and do it right. You're 19 with test levels of 8year old. So be safe and thoroughly study up trt as its a lifetime of keeping it level.
Any prohormones...steroids priors? Weed? Meth??
