+ 2 Help with Bloodwork Please

ad

If some of you guys could give me some help with what I should be looking for in my forth coming visit to an endocrinologist it would be most helpful.
Certain things look a bit out, the brackets contain figures which I assume are the normal range for someone my age (58 in a few days)
So
Liver function test- Outside normal range but no action needed
! serum ALT level 591 iu/L (<41 )

Thyroid function test – Normal no action
Serum free T4 level 14.6 pmol/L (12-22)
Serum TSH level 2.07 mu/L (0.27-4.2)

Serum Electrolytes Normal – No action

Serum testosterone 4.16 nmol/L (9.9-27.8) Low

Full blood count – Outside normal range but no action needed, please note new adult MCV reference range
Platelet count 532 109/L (130-400)
Monocyte count 1.3 10
9/L (0.2-0.8)

My untutored thoughts
Testosterone seems very low hopefully TRT to be given
Serum free T4 at the low end any T3 or T4 supplementation?
No idea what the rest is or implications, curious though if anyone can shed any light on them.

Just an update with follow up blood test.
Follow up blood test 32 days later shows my ALT level reduced from 591 iu/L down to a normal range of 24 iu/L. I’m not sure why the excessive reading was but not drinking enough water would likely not have helped. Thanks everyone for your previous input I’m relieved to be back in the safe zone again.

Great news! I gotta PM you to see how you're doing in your own adventures. However, it's always nice to be "normal" (regarding health, of course).

Med has you all worked out down below. Take his advice.

I believe u caused some damage to ur liver as evidenced per ALT on bw. If ur not yellow, thats good, but ur not outta the woods, either!

I would like to see the complete lab data hard copy, if possible. If not, maybe u can inform us of other values that u say are okay, or in range.
BUN/Cr....HGB/HCT...etc..

Diet needs to focus on moderate carbs and fat intake. You can eat protein, but not a lot.

I've never seen elevated LFTs induce jaundice before.. that's always been related to elevated bili. Say that high ALT was related to HCC or something of the sort and his biliary tree was obstructed then he could be yellow ;) but would still have an elevated bili. Sounds like we work in a similar field. FR sent bro!

Do u agree with plan advised for OP? I am not aware of any other OTC Rx that OP could benefit from.....or for that much what an MD could do without admitting him.... :/

Gh0st's picture

Absolutely. There is only so much that can be done OTC. I think what you suggested is a good idea. OP needs to go see his doctor. Will most likely be referred to GI to follow-up on the elevated ALT, with a full hep panel and other lab work, as well as an u/s of the liver. Hem/Onc follow-up for the platelets, or something as simple as OTC ASA treatment and watchful waiting. That's just my best guess. But OP hasn't led me to believe he's not stable right now, so I can see all of this being done outpatient without admitting him.

Yea def outpatient...nice

MedDx's picture

Thats true...ur right...i was painting a more chronic condition...this seems to be acute and hopefully reversible with minimal Rx and no long term effects or damage. Thanks.

Indirect or direct bilirubin?

FR accepted....

Gh0st's picture

I hope so for OP sake. Sent you a PM to discuss further

U dont have a BUN and Creatinine level?

Are u takin any other meds?

Do u have any drug allergies?

MedDx's picture

Thanks...follow my suggestion...stay hydrated with water and drink 6 liters per day.

Ur sodium and potassium are headed the wrong way too...

Last Chance57's picture

Thanks very much for your help.
I'm not on any other meds and have now got milk thistle and liver support capsules alongside vitamin/mineral tabs.
I should also point out that I'm getting a fair ammount of business related stress at the moment which probably doesn't help.
I do however feel ok and have increased my water intake not sure if i can manage 6 litres a day but will do my best.
Very much appreciated help
Thankyou

Did you just finish a cycle? Are in the middle of one? Moderately high platelets aren't usually a big deal but do carry the increased risk of thrombosis. Monocytes could be elevated if you're just getting over a recent illness, probably not a big deal. Is this bloodwork from your doc? Why dont you ask?

If that ALT is actually 591 and not a typo that's no good and needs to be looked into.

As far as your low test idk what were your other numbers? LH? FSH? SHBG? E2? Etc?
You probably wouldnt need any thyroid supplementation your TSH is fine.

Thx for clarifying ALT. I read it as 59... :-(

These weren't given in my blood test results, maybe the endo will want further tests?

Hi Switch87x
Just finished a 6 week course of oral steroids,I posted the cycle on my thread 6 months to live (dramatic I know but I was really quite desparate) clean for 48 hours before blood test, sedentry and overweight lifestyle before recently begginning training again.
The ALT is not a typo, I have an appointment to see an endocrinilogist 18th december so will ask him about my results then as in the UK its not easy to get a doctors appointment usualy takes a couple of weeks near me unless you are really ill that is.
What does the ALT mean?
Glad to hear my thyroids ok but feeling a bit more tired now I'm off the steroids.

MedDx's picture

What is the name, dosage and frequency of oral steroid used for 6 weeks?

If i were you i would get TUDCA ordered ASAP. After that, go to wally world and get mlk thistle and Liver Aid, TODAY. Start those as directed per bottle ASAP.

I read ur ALT as 59, yesterday. My bad, bro! ALT stands for Alanine transaminase.

Endo will refer to Hemotology with elevated PLT and ALT.

MedDx's picture

I believe once u start the liver supps your platelets will decrease, followed by ur monocytes....while ur ALT decreases as well.

Last Chance57's picture

OK so I’m off the juice a bit earlier than expected for blood tests for HRT.
What I actually took was
Week 1 5mg Dianabol2 (2 means twice a day 10mg total)
Week 2 5mg Dianabol3
Week 3 10mg Dbol in morning and 5mg Dbol
2
Week 4 20mg Dbol in morning 15mg Dbol 2pm and 10mg dbol 8-9pm
Week 5 add 125mg Proviron morning and 140mg Andriol morning and 2pm
Week 6 add another proviron in the evening
Blood test next week for HRT so Friday night last Dbol, continue Andriol and proviron till saterday night then off for bloods Monday.
I'm not yellow and don't feel too bad, a little drained maybe.

MedDx's picture

Dont take anymore oral AAS.

MedDx's picture

Get a liver supp on board asap....TODAY, if its the first thing u do!! Smile

boostedsilvia's picture

Yea your platelets and monocytes are off by quite abit.

Last Chance57's picture

Thanks boostedsilvia, I'll ask about them at my next doctors appointment.

MedDx's picture

Query elevated platelets and monocytes..

Last Chance57's picture

Thanks MedDx, I was hoping you'd reply as I've read some of your other posts. I will ask about them on the 18th december meeting with an endocrinologist and post anything that comes up here, hopefully some other poor sod can benefit.
For me at 57 the low testosterone the worst thing was the chronic tiredness I could probably live with the sexual disfunction although its better not to and seems treatable. but it came on so gradually I was undiagnosed for about 4 years.
Thanks for your reply.
