I've seen a lot of these over the years. It's no cause for concern. You can make the top of the stopper as custom as you want. What's important is the flat lip on the underside of the stopper that makes contact with the top of the vial. That and the actual part of the stopper that plunges into the vial, the fit should be snug. The machines that make these have really good QC, so there are automated mechanism in place that automatically discard any stoppers that are out-of-spec.

So the thing of it is that the stopper seems to have been so over crimped that it actually pierced
Although it resealed as a good rubber stopper will do, it was definigely damaged with the inside sonewhat soingey, presumably damaged by the contents
Yes, bro, not important where is the stopper deeper or bulge, contact is the main, but, this stopper looks strange. Or vial under pressure or rubber was bigger than necessary or aluminum retainer larger in diameter. But the center of the rubber looks like damaged. That's not good

I opened older vials a few months ago and all looked like yours and this from from bearded_muscle.
