TRT injections and results
I am posting to share my recent lab results and consult with members regarding my current testosterone replacement therapy protocol.
Two years ago, I began treatment with a baseline testosterone level of 298 ng/dL. Initially, I was prescribed 1.6 cc of 250 mg testosterone cypionate once per week on Mondays. My blood work under that regimen showed a trough level of 709 ng/dL, but I experienced significant energy drops later in the week.
To achieve more stable levels, I transitioned to self-administering testosterone cypionate (200 mg/mL) at a dose of 0.75 mL twice per week, specifically on Monday mornings and Thursday afternoons.
My recent lab work from July 1, 2026, shows the following elevated levels:
- Total Testosterone: >1500 ng/dL (Reference Range: 264 - 916 ng/dL)
- Free Testosterone (Direct): 47.7 pg/mL (Reference Range: 7.2 - 24.0 pg/mL)
- Estradiol: 50.8 pg/mL
Since switching to this protocol and experiencing these higher numbers, I have noticed a significant drop in my libido.
Given these results and symptoms, I would appreciate your guidance on whether my current levels are unsafe and if I should decrease my dosage. Please let me know your opinion or share advice of the best next steps for reviewing my protocol.

Call me crazy... but I don't understand this huge swing.
The doctor had you pinning 400mg a week & your levels were in the 700s, then you pin 300mg a week (split 150mg/150mg) & your levels aren't just higher, but at minimum double???
So to explain. After 2 years at the mens health location I visited, I was given 1.6ml of 250mg of TCyp every Monday weekly. The trough is what killed me.
Prior to injection every five weeks, they would draw blood and test my T levels on average I ran 750ng during trough.I went the Dr there and said I need to split my injections into 2 doses and he said we can't do that. So I said bye,bye and found a supplier of 200mg of Tcyp. I talked to a couple friends and they said they had been running 2cc per wk split into 2 doses. I ran 2cc of 200mg injecting 1cc Monday and the other 1cc on Thursday eod. After getting blood work done they provided paperwork showing greater than 1500ng of Testosterone. I couldn't figure out how it was possible with a lower milligram dosage. The math said add .25 to make up for the difference in MG. I felt like the Low ? Center was pulling a good one on me. Since last blood draw I have slowly lowered my injections and will continue to do so until I can get somewhere close to 900ng. Hope this helps.
You shouldn’t experience a trough that would make that huge of a difference. Many guys are switching to 2x weekly pins for TRT, but I have never experienced a swing which you describe. I would assume you will be good somewhere between 100mg-150mg. 125mg is putting me in the 700s, however I am currently reevaluating my dosing right now as I just got on pharmacy grade cypionate which I hope is more accurate than some of the guys on here. This is the way. 7-800 range is such a sweet spot. High enough to feel great and little to no side effects. And when you blast you actually feel it.
Go from 25yrs old feeling to boosted. Wooo!

Running high seems like the fun road but once you're outta that natural environment too long it catches up. Im at 800 and its been like 25 for life. Then blast 1 or 2 times to add a lil dimension as needed.

Get your shbg and hematocrit checked. Your free test should be higher IMO. And hematocrit should always be monitored, can help determine if dose adjustments are needed. If you already have those numbers, post em

You may not need one if you cut back on the dose some. Get yourself in the 7-800 range.
I use the same rec for everyone in your situation, start at 100mg a week. Check bloods, and adjust. It can take a while to find your real trt dose, but its worth it for the long run
