HDL suggestions

Hi all,

Currently running 350 test e, 250 primo, and on my final weeks of 70mg tren ace, eating in a deficit at around 2400kcal/day (200-220 carbs, 200-240 protein, >50 fats) as I cut. Got bloodwork yesterday and everything looks good except HDL. Hematocrit at 42%, ALT and AST in the 40s (on a training day), GGT at 18, LDL at 80, ApoB at 85. But my HDL is tanked at 25. It seems whenever I run my test over 300 it dips into the 20s, but never into the teens.

I’m on 2.8g of fish oil (may need to evaluate brand and quality - it is the Kirkland brand salmon oil, with 300mg DHA), citrus bergamot 300mg, I get 12k steps daily (30min after breakfast, lunch and dinner) and zone 4 cardio 3-4x a week. Any suggestions for improving HDL on cycle that I might be overlooking? Thanks!

Just keep ldl, apob, and triglycerides low and you’ll be fine. My HDL was 25 before I even started gear. Have had genetically low HDL my entire life.

Overall, your labs actually look pretty solid for someone running a cycle. I’d be more reassured by the ApoB of 85 than I’d be alarmed by the HDL of 25. ApoB is generally a better predictor of cardiovascular risk than HDL alone.

That said, an HDL of 25 is common on cyvle especially with higher testosterone doses and trenbolone. It’s often more a drug effect than a lifestyle issue, and it may not be fully “fixable” until the cycle changes.

A few things I’d consider: Reduce or discontinue the tren if you’re at the end anyway. Tren is one of the harshest compounds for lipids. Evaluate the fish oil. Salmon oil isn’t the same as a concentrated EPA/DHA fish oil. I’d look at the actual EPA + DHA content rather than the total oil amount. Increase soluble fiber (psyllium, oats, legumes) if your diet allows. It won’t dramatically raise HDL, but it supports overall lipid health. Double-check citrus bergamot. Many studies use around 500–1,000 mg/day of a standardized extract rather than lower doses. Make sure dietary fat isn’t too low. If you’re truly only getting around 50 g/day while cutting, modestly increasing fats from sources like olive oil, avocado, nuts, and fatty fish may help, although the effect on HDL while on cycle is usually modest. Avoid adding niacin just to chase HDL. While niacin raises HDL numbers, clinical trials haven’t shown that pharmacologically increasing HDL improves cardiovascular outcomes, and it can have its own side effects….and you should be using Time/Slow release niacin NOT the stuff you buy over the counter. Honestly, I wouldn’t lose sleep over HDL in isolation if: ApoB remains <90 mg/dL, LDL stays well controlled, blood pressure is in range, Hematocrit remains controlled and gluxose and HbA1c are normal.

If HDL stays in the 20s months after coming off or after returning to a lower-dose regimen, then I’d investigate further. But while on 350 mg test plus primo and tren, it’s not an unexpected finding.

I also think it’s worth remembering that the cardiovascular risk from steroids isn’t explained by HDL alone. Blood pressure, ApoB, lipoprotein(a), insulin sensitivity, inflammation, cardiac structure/function, and cumulative lifetime exposure all contribute to overall risk. Chasing an HDL number while ignoring those other factors is probably focusing on the wrong target.

I'd 100% 4g of fish oil and atleast 1200 mg citrus berg. Never dealt with tren but that shit 100% fixed me up while on dbol for 5 weeks.

I’d swap most of that Zone 4 for Zone 2. Four to six hours of Zone 2 per week has much better evidence for improving HDL and overall cardiovascular health, and it’s much easier to recover from. I’d keep Zone 4 to one session a week at most unless you’re specifically training for performance. I’d also consider increasing your combined EPA/DHA intake to around 4-5 g/day from a high-quality triglyceride form fish oil. With an LDL of 80 and an ApoB of 85, your HDL is really the only thing that stands out.

Thanks for the suggestions! I'm getting 1900mg EPA/DHA from my current fish oil, so I should probably get one with higher DHA content. I do a lot of Zone 2 (probably 4-6 hours a week total) but was adding in some additional Zone 4 primarily to keep my cardiovascular conditioning while running tren. I appreciate the suggestions!
