+ 1 Ready for August 1st Cycle

After reviewing the my lab report, my overall assessment is my blood work is in excellent metabolic health and exceptional physical condition, with one clear area that deserves attention: cardiovascular risk related to cholesterol particles (ApoB/LDL). The rest of my labs are remarkably good and feeling good about my August 1st bulk cycle
1. Excellent metabolic health
This is probably the strongest part of my report.
Fasting glucose: 80
A1c: 5.2%
Triglycerides: 81
HDL: 54
excellent insulin sensitivity, no diabetes, no prediabetes, no metabolic syndrome..Many people with elevated LDL also have high triglycerides and low HDL. I do not, this is good

  1. Kidney function looks healthy
    Initially I was mildly concerned by: BUN: 30, BUN/Creatinine Ratio: 28,
    But once I saw: Creatinine: 1.08, eGFR: 81, Cystatin C: 1.00 and Cystatin C eGFR: 79
    my concern essentially disappeared. Why? Because Cystatin C is much less affected by muscle mass than creatinine. Both kidney filtration estimates are essentially identical. That strongly suggests my kidneys are functioning well. The elevated BUN is much more likely due to: my high protein intake, mild dehydration than any kidney.

  2. Liver looks excellent
    This was another area I liked. AST = 39, ALT = 38, GGT = 18…This pattern is classic for someone lifting heavy and I am….If the liver were the problem, I would expect GGT and/or ALT to be elevated as well. Instead: AST is barely above range. GGT is excellent. This strongly suggests skeletal muscle rather than liver injury.

  3. CBC
    Everything looks outstanding except one value, Hematocrit at 51.5%..Slightly elevated. Given my testosterone level: Total Testosterone = 904, Free Testosterone = 192.9, this makes complete sense. I generally begin paying closer attention when hematocrit approaches 54%. At 51.5% I need to make sure that I’m staying well hydrated, continue monitoring, repeat in a 8 weeks after my cycle begins.

  4. Hormones
    Overall...I’m happy: Total Testosterone 904, Free Testosterone 192.9, Above the laboratory reference range. Not surprising given im cruising on 150 Mg/week.
    SHBG 34, Perfect. DHT 43 Excellent. Prolactin 10 Perfect. DHEA 205 Excellent. IGF-1 244 excellent, Z-score 1.3, This is right where I'd want to be as I focus on muscle growth and longevity.
    Estradiol 58, Higher than the lab reference….However...Considering:Testosterone = 904, my testosterone-to-estradiol ratio is approximately 15.6:1. I have no gynecomastia, no edema, no mood issues, no libido issues. I do NOT automatically try to suppress estradiol based on the number alone. My decisions about lowering it is always driven by symptoms and the overall picture, not the lab value in isolation.
    One issue is my Cholesterol….This is the only part of the report I need to actively work on.
    Total Cholesterol 223, LDL 151, Non-HDL 169, ApoB 125…..Those four values are all telling the same story. I have more cholesterol-containing particles than I'd like to see. ApoB is the most important number. This is the ONE abnormal lab... It's this. 125….this tells ne that there are more LDL particles circulating than ideal. I am having a CAC test done it’s scheduled for Setember. I am NOT panicking because my risk profile is unusual. I have ecxcellent glucose, excellent triglycerides, excellent HDL, excellent blood pressure, excellent weight, excellent fitness, no smoking and no family history of premature heart disease. I’m not the classic high-risk patient. The question now becomes has my elevated ApoB actually caused plaque? That is exactly why I am having the CAC scan.
    My friend is a doctor and he think is most likely my elevated LDL/ApoB is probably due to a combination of: genetics, diet (particularly saturated fat intake), and/or TRT effects.

Have you considered Pitavastatin, ezetimibe, and Bempedoic acid to keep lipids in range? Would definitely recommend them. Also, if you can get repatha covered by insurance that would be even better.
