I'm gonna write this because I hope it might help someone.
A little disclaimer upfront, English isn't my native language, I'm not an expert, and I'm fucking tired while writing this, so spare for me any little mistakes.
If you are on TRT or planning to do so, make sure you have done your bloodworks with a full thyroid panel;
TSH
T3/T4
FT3/FT4
Reverse T3
Antibodies
TBG (when elevated, it renders your T3 inactive)
Why do i say this? Because standard checks are limited (usually in the WEST) to just TSH and FT4, and that is just not enough, they might give back results in the normal reference range...
Most people will know or understand that T4 converts into T3, but people that have (elevated) TPO antibodies in their blood, could have problems with that conversion (High reverse T3).
Those antibodies are attacking your thyroid, this is an auto-immune problem (Hashimoto's / Graves). Your auto-immune system becomes overactive, this makes your body (and mind) be in a constant stress (high cortisol) and that stress will render your "available" T3 inactive, so you will still feel like SHIT.
Thyroid problems or auto-immune problems overlaps with symptoms as LOW-T, hence why I'm putting it in this section.
I titled it "The Silent Killer" because this problem might take many years to really develop.
Usually doctors check TSH, and only after it becomes above the reference range, then they will check for antibodies, and then they say "Oh it's Hashimoto's". But antibodies might be present already and could take up to average of 10yrs to kill your thyroid, and it's slowly killing mine, and took many years of my life to figure this out.
Indication for me was that i always had elevated Prolactine levels, which then let me check to thyroid problems, and looked into labs i had done with multiple endo's and in trt clinics in past, and only one had my antibodies checked, which was slightly elevated back then, but it's very high now.
I've had many medications, tried so many therapies, nothing helped me, until i figured this out and started using some T3. After 3 days of taking 0.25mcg i already felt my body and mind started to work again and knew I'm in the right direction.
Have an appointment with an endo next month, so i stopped using it, then i won't skew up the bloodresults, and have a nice baseline again, and hopefully some free T3 meds

I found this article that says that T3 also acts an aromatase inhibitor, this is new for me.

My wife has hashimoto's, armor thyroid worked for her. Leaky gut due to years of bad eating and gluten intolerance. When your reverse t3 gets too high it basically fills the receptors that the t3 should go into. Tsh is fine, but a full panel and symptom recognition is needed.
Diet, diet, diet. Address and fix the problem where it starts, the gut. This is a slow process and takes time. Certain auto immune paleo diets are recommended. I've lived this hell with my wife a long long time.
A hell it is for sure, hope so is fine now!? Is she still on Armour Thyroid?
I've read that NTD's are usually not the best option when having antibodies, as the antibodies can also attack the natural hormone products
Gut problems are often caused by the lack of stomic acid, has she ever tried Betaine HCL & Pepsin supplements? This could solve that problem...
Yes, she is still on Armour. She is really just bad with gluten. When her diet is good everything is fine. When shes off the wagon, really bad. She's tried tons of stuff but nothing like you mentioned. I can only help so much before I become an aggravation to her. Synthroid had very little effect for her symptoms.
Betaine HCL & Pepsin is an "all-in-one" supplement, got some good reviews from various brands.
The Armour Thyroid has some T3, thats whats makes it a little better for her i guess.
In the past, standard protocol for slow thyroid was a T4/T3 combo (4:1 ratio), untill big pharma start pulling strings on T3, and raised the prices like crazy in the EU/USA. You can buy it in most countries for pennies over the counter.
Now is standard protocol the cheap T4, and with some luck an NTD or very little T3.
