It's crashed, simple fix, put it on a towel on it's side and hold the hair dryer to it and rotate it little by little, you will see the hormone melt back into the oil...
It should be fine after being reheated (it happens more then people realize). After its back in suspension it should stay that way as long as it's stored right. If it crashes again let us know.

Wrap it up in a heating pad on high. Swirl it and repeat if necessary.

once it re-suspends yes it will be fine to take. Make sure you get all teh crystal's dissolved
