High cholesterol Triglycerides only on TRT for little over a month plus other concerns
Sorry for long post but im trying to provide as much complete and thorough info as I can.
In feb 25 I started taking 200mg/wk Test C due to all the symptoms of Low T. I had been on TRT before by using the pellet therapy but was just way too expensive so I had to stop so for about a yr I was off Test all together. In jan of 25 I had bloodwork done by my Dr, who I mentioned I was on the TRT pellets previously and had to stop and asked for all the levels check and they refused and said I only need to check total T which came in at 255 which according to them was norming, when the low range was 250. I kinda knew they were full of crap and the fact they wouldn’t check free t, bio available, shbg and all the other tests told me all I needed to know.
The blood work from January 16 showed my cholesterol with the following values. ( added pics of full lab)
CHOLESTEROL, TOTAL 146 Reference Range: <200 mg/dL
HDL CHOLESTEROL 38 L Reference Range: > OR = 40 mg/dL
TRIGLYCERIDES 138 Reference Range: <150 mg/dL
LDL-CHOLESTEROL 84 mg/dL (calc) Reference range: <100
CHOL/HDLC RATIO 3.8 Reference Range: <5.0 (calc)
NON HDL CHOLESTEROL 108 Reference Range: <130 mg/dL (calc)
A1c in jan 16th was 5.6 Normal value: <5.7% of total hgb
I had bloods done in feb after just 3 shots of test c at 200mg/wk but forgot the lipids also forgot A1c and there are pics of full labs here as well
Now I had bloodwork done yesterday 3/19 and things are way out of whack. Especially Cholesterol /Triglycerides (full report minus Total T, Free T, Bioavailable T, Shbg, Albumin in pics)

Consistent 7 days a week, usually 2 rest days but those 2 days I do 30-45 mins of cardio no lifting
5 day lift end with 30 mins of cardio

Meal 1: 4 eggs, 1 serving protein pancakes

Meal 2 : bfast wrap, 1 egg, 2 oz sausage, ¼ cup fat free cheddar 0 carb tortilla
meal 3: Double protein shake w 1 banana 1tbsp Natural PB
meal 4: 6oz top round leans teak, ¼ cup rice 65g sweet potato
Meal 5: 6oz 93/7 ground beef 65g sweet potato, ¼ cup fat free cheddar, lettuce
meal 6: 6 oz chicken breast ¼ cup rice 65g sweet potato
Cals: 2528 Protein: 210.3g Carb:170.4 Fat: 81.6g
There is very light snacking here and there and by light I mean maybe a cookie or 2, or a single reeses mini egg before bed, nothing crazy or out of control
Cheats are usually Saturday night/Sunday and by that I mean ½ sub (usually meatball or cheeseburger sub) sat night, Saturday during the day is usually meal 1-3 or 4 from above, Sunday breakfast sandwich then other ½ sub late lunch around 2-3 pm and then Sunday night ill usually have a pint of ben and jerries then back to normal diet above on mon-Friday
Diet and gym has been consistent like above since Dec 24/ Beginning Jan 25 Currently 340lbs and have been since December with no change, bf 28% and been the same since dec24.
With everything going on here. Lipids whacked suddenly, and the Estradiol high low LH, where should I go from here? Add the AI in? add HCG? Do I need to get statins? Could it be a bum test and check lipids again to be sure? Do i lower T dose and add HCG ? So many variables to consider.

Hey brother…no shade from me at all. I understand food addiction. I’m a couple inches short than you (5-11/48yrs old ) and before i transformed my physique a couple years ago I weighed 270. Your home scale is garbage and way off. I did a dexa scan when I started and I was %33BF.at my stats. At 6-2/340 you’re closer to %40…No offense. I also thought about getting on test when i started my health journey. My HRT doc (who’s a personal friend) told me these exact words, “until you learn how to control your diet and train properly, you’ll be a fat guy on TRT.” So, like you I got on Tirz for 5 months. I lost 40lbs. I got off the GLP and lost an addition 15lbs on my own. At that point, I hopped on TRT. So glad I waited bc as the science shows, anabolic androgens shine the leaner you are. I’m now 210 at 9% BF per dexa. It took almost 2yrs. So don’t rush it, and do it right. My blood work is perfect. I’ve done two mild blasts and my lipids recovered swimmingly. Do what you want but losing bodyfat comes to down diet, calorie deficit, an GLP and training. Not steroids/Test
Fasted cardio and listen to the guys, they gave you solid advice
To me, I find it hard to believe you’re doing that much cardio and still staying at that weight. That tells me you’re either over eating or not doing the cardio you say.
Two simple options to fix your issue- dump food or do more cardio.
30 mins daily after every lift session and rest days 30-45
However I do use stationary bike.
The point of my post is not weight loss . I keep tinkering with my diet till I find the right combo and my current diet above in one week lost 2 lbs so that’s not my main concern yet.
the point is my lipids were fine in January with worse of a diet than now and trying to figure out if it’s caused by the test c or not

More of being very unhealthy/obese and on lots of medication than the TRT I’d say.
Those cookies and reese’s “small cheats” are 600* calories you are just letting go in the wind. Probably 1-3lbs a week adding easily…
Cheats are for when you’re in shape and trying to keep on track.
You need to drop all the bullshit and actually lose the weight.
Eat healthy and log everything. EVERYTHING!
Cardio.
Calories in vs. calories out.
That’s the bottom line no matter what anyone says.
Get that willpower trained.
Cold showers. Limit meals. PUSH yourself.
Get comfortable being uncomfortable.
Those small cheats aren’t every night /day they’re literally just here and there
The point Is my lipids were fine even on a worse diet than above . And now are whacked out.
The my above diet this past week was down 2 lbs.
So what I’m trying to do to a find why things are high now in a better diet . Than when my diet was absolutely trash daily and lipids were good.
I think you have to be honest with yourself here and really look at what you are eating everyday, there is no way you can be eating healthy and not consuming much fatty food if you weigh 340lb's. Being on only TRT its not as if a high proportion of that is going to be lean muscle either - so its going to be bodyfat. I have just had bloodwork done and yet again my triglycerides and cholesterol levels have come back in an extremely good range, the Doctor can never believe it given the fact I pump steroid oils all day long. Now although I do consume a lot of carbs and moderate protein, the food I eat is extremely low in fat. So never steak or things like that. I often eat pretty feeble small portions too, just quite often. I never do any cardio either - so it simply has to be down to diet. As other guys are saying also it looks like you are going way overboard on some of the supplements you are taking and that could be having a detrimental effect on certain things too.
WHat in my suppliments is over kill? Ive always been told to take DIM with Test, even when I was with a clinic and did test with them using the pellet form. Really only suppliments are the multi the coq10 and the cycle support which is an all in one thing. Figured take that to be proactive with the organs and it cant hurt. Especially as my liver and kidney levels have been great Ive taken it alone with no test as well
What i have above is what I eat everyday. I prep everything sundays all weigh it all out. Ive learned alot of impulse control over the past few years so when I say light snacking I do mean it, i dont gorge out and then try to justify it. or come on here and say i have just 1 of these when really I had 5 cause that defeats the p[urpose.
The diet I posted is something ive done before with general good results, but i just never seen my lipids get like this, even when I was a super heavy drinker. So it seemed off to me. Even when I was on test before with a clinic and they did it via pellet method it didnt jump like this .
IM always looking to imporve the diet however every couple of weeks I crunch numbers out and see where I need to make changes
Also what is your training split and are you progressing in weight/reps over time or you just going through the motions doing fluff workouts and be honest.
Training is progressing good actually weights and reps vary. But it is progressing . I'm using old workouts from a coach I had where each week is different, 1 week could be 4-5 sets of 12 plus reps some weeks are 4-5 sets of only 7-10 reps. Usually it goes legs, chest, rest(but i do cardio only), back, delts shoulders, arms rest or some variation . They're fairly good workouts and I have had success before. And since getting back to gym since december ive progressed. to answer your ? I am 6'2, and im very broad, it doesnt look like im 340 no one even believes it when I say it. But i have a history of being in the gym, since I got really serious about it in like 2012 there was one yr i was off in like 2018 due to poor life decisions, and then I was off from like end of 22 to 23. I got back in around mid 23 and around march of 24 I stopped till december of 24 again poor life choices. So my journey didnt just start in dec of 24 i just had to get my self back into the grind. As for the Tirzepatide my Dr wanted me to try it where in 2023 i tried wegovy and didnt do much for me but i wasnt training or dieting very well. So I figured ill try this now .
Also walk 10k+ steps a day on top of your cardio!
Ok I’m 6’3 and weighed 300 at my heaviest 5 or so years ago. I personally don’t have much experience with GLPs so I can’t say much but I’ve heard and know a lot of people who have lost weight utilizing them. I agree with a lot of what everyone is saying in terms of your diet as you can be strict Monday through Friday eating in a deficit and fuck it up on the weekend as it’s the spread through the whole week. I’d also pay attention to season, oils and sauces used while preparing your meals as they can add plenty of calories that we wouldn’t even think about. I would look in to Cronometer, it’s like my fitness pal but tracks vitamins, minerals, micro and macro nutrient which will help you dial in your diet which in term will help you in the long run. Now for your cholesterol as that’s what you’re worried about, Test can throw off your lipids especially if your diet is off. So I would dial in your TRT dose start at 120-150mg, see where E2 and everything is and make sure there in range. I would supplement with fish oil, I eat a gram of EPA DHA with breakfast and lunch and also look into Citrus Bergamot as I always use this with my TRT and helps me say in range. In the end diet is always key as supplements are amazing when included with a healthy diet. You can’t put a bandaid on a gunshot wound!
I’m sorry Imma ask you a bunch of questions first then I’ll give you advice. What is your height? How long have you been on Tirzepatide? You haven’t lost any weight since starting your journey in December? Do you have an office type job or are you moving a lot through out the day? What are your goals with all of this?
Ranger3252Crushing 10,000 mg of fish oil a day will obliterate triglycerides. It’s facts and proven. Mine are less than 11 every blood test. Do that for a month and get bloods I post me reputation on it.

Anvil said it best. Question, the naltrexone, are you in substance abuse recovery or are you supposed to be using in conjunction with buproprion for weight loss? Also, the amounts of supplements you are taking is extremely high. IMHO, I would look to diet first and foremost until you achieve at minimum 15% bf and blood test again before going to all of these extremes. The bf % and the overdosed test is worrisome on multiple levels. I would definitely drop down as Big Tone said. The supplements are there to supplement, not be the main driver. It seems to me as a prolonged weight loss 16ish weeks would be a good starting point dropping 1-2 lbs max per week. But then again tell me to stfu lol. Best of luck.

I was on it for a year using the vivitrol shot which got old, then stopped it. then I noticed my drinking getting heavy again so I went back on it just in pill form. since about october I barely drink. Ive had maybe 2 drinks in 2 months total
As far as diet thats what I have planned out above after crunching number and maitenance cals and tdee and all that stuff . That diet usually has never been an issue for me
Also, if you're concerned with supps and want to continue with trt, that's cool but again drop the dose and definitely add retatrutide, tirz, or semi. What's the workouts like? cardio? I'm in recovery myself, so no judgement. But you can't drink at all and lose weight. Period. And also the snacking and cheat days and ice cream gotta go. None. The supps don't hurt at all, but I feel like you are more concerned with them than what actually will help you lose bf, which is diet and mild cardio, and NEAT. The test is definitely too high and you run risks of major health problems at your bf, not trying to be a dick but helpful. I think really dumbing down the diet even further, chicken breast, rice, some green veg, lean turkey, oats, real simple shit. Same thing every day. No cheating. Increase your steps everyday to at least 12k.
anvilIt's because you are still unhealthy. I will put this plain and simple. Cheat meals and justification for snacking are for healthy individuals, not 340lb people. You have 100% dial in, you have a page long list of drugs and weight loss drugs and you still are holding on to bad habits.
Achieve good health then enjoy cheat meals. This may sound harsh but check egos at the login screen. Steroids are going to make it hard to keep lipids in healthy range in the first place. You need to up the cardio, reduce the cookie and chocolate mini eggs to 0. Best of luck and health to you.

If I have crushed your dreams apologize, getting healthy is not easy and you forgot Saturday/Sunday cheat meals of subs and meal 1-3 on one of them. This bothers me a lot, nobody in this mental state needs more drugs (steroids) on top of all the other medications, when your goal is to be healthy then you can incorporate cheats in. At 340lbs states he is only consuming 2500 calories which is close to half the maintenance calories and 28% sounds all wrong. He would be dropping weight like crazy, and I would bet BF is higher. 10% deficit I start dropping, and he's around 40%..I don't need to complete detective school to know something is off here.
Yea man. Diet can be dead nuts on all week. But 1 or 2 days on the weekend of cheat meals just ruined all that discipline during the week.
His maintenance calories would be around 5400. So 2500 calories daily he would be dropping over a half pound a day for the first 30 days.
Most people don’t realize or think about how that works when dieting. Nowadays a satisfying cheat meal is a couple fat ass meat sandwiches. Hella meat 2 slices of Swiss cheese and 4 slices of white bread. But say I went to McDonald id get a double quarter pounder extra patty 2 mcchickens 10piece nuggets and fries. Well there goes weeks of dieting lmao.

Up you Omega 3 to 3000mg daily, stop eating shit food and un healthy fats. Do cardio. And you should be taking more like 125mg per week not 200mg
First, this is a bit of an overreaction to one test. Keep an eye on it. You can go years with high cholesterol without it being a problem.
Did you do your bloodwork fasted? If not, that's your trig value.
7.5 mg of zepbound per week is a pretty hefty dose. 340 lbs and only 28% bf? Doesn't sound right, sorry. Have you considered a keto diet? High trigs are most commonly because of carbs. So be more honest with yourself about your "light snacking". At 340 lbs and that much tirzepatide, you shouldn't be snacking. Period.
I'm also assuming your "TRT" is not pharma grade. Why do you think you need 200 mg/week? This is already a pretty high dose, probably the highest that is considered "TRT". In reality, you probably should be around 120-150 mg/week. Consider that your UGL test is possibly overdosed, and now you're taking an already high dose, even higher. That scenario would explain much of this bloodwork. Drop it down, wait a few months, test again. Don't go changing too many variables all at once.

The tirz was recommended by my doc because I tried wegovy in the past but did nothing but also wasnt really dieting right and training right at the time so Im not blaming the wegovy. SO I figured id try the tirz while im in a good / ok spot and see if it helps

Keto also doesnt work well with me, something up crazy high fats does not agree with my internals at all, even to the point where ive felt like I had kidney stones, which I was told is im not [producing enough bile to digest all the fat properly, and was recommended a probiotic with ox bile and it def worked, but the keto way was never for me. Hence the sweert potatoes and rice.
