+ 5 Don't be a PIP squeak. (Tips for newbies on Injections, to prevent PIP)

ad

Some tips for newbies on injections and preventing PIP (Post Injection Pain) that I wish I would have known when I first started:

1) If you're new to needles, stick with 25gauge or backload your gear into slin (29g insulin) pins. 23 gauge isn't for everyone.
2) Never, ever, use the same needle that you use to draw from your vial to inject with. You dull the needle as soon as you poke anything with it. The stoppers on vials are not cotton. They're very durable.
3) Massage the muscle before and or after an injection to loosen the muscle up, sometimes our muscles are tighter or just not "primed" for injections, especially for some upon waking up when your blood circulation is slower. If you don't know what to use to massage the muscle, a rolling pin can do for quads, or a foam roller, or really anything. For myself, massaging the muscle post injection has helped tremendously in reducing discomfort/pip.
4) Don't ever store your gear in a cold place, unless it's supposed to be stored in cooler temps. Your gear should be at room temperature.
5) Many believe it helps to run a vial, satchel, ampule, or even the syringe under hot water to make the oil a bit more thinner and easier for the muscle to absorb
6) Don't rush and push all the oil in as fast as you can because you have a needle stuck in your leg/arm/glute. Take your time and slowly inject it at snaillike speed. I'd say between 30-60 seconds is ideal for 1 ml of oil.
7) Finally, know where to inject. Everyone has their preferences, but know that wherever you pin, the muscle gets used to it over time.. But you should still always rotate.. ie: right quad, left delt, left quad, right delt. It doesn't really matter if you stick to say left quad, right quad, left quad, right quad too much as long as you're not pinning in the same area consecutively.

Other tips: Don't skip on the alcohol pads. Very important to keep things clean/bacteria free. Alway swab the top of your vials, and your injection site. Always aspirate when injecting to make sure you don't hit a vein/vessel. Dispose of your needles safely using a sharps bin. and don't go stealing a diabetic patient's insulin pins.

Here's a helpful resource that's been around for a very long time now that has good pictures/diagrams on spot injections and other useful info: www.spotinjections.com

Never heat ur gear under water always use a heating pad also I wouldn’t message after u pin again I would place a heating pad on the injection site helps lesson pip IMO. Fuck slin pins unless u pin constantly and use smaller muscles just throwing my thoughts out here bro ur heart is in the right place man.

Yea I def wouldn't run water over my vials lots of bacteria better to use a heating pad if need thinner oil. Never have massaged the muscle either just more bacteria and scar tissue . Everything else helpful for newbies

I would recommend not to massage area after injection. Moving that bolus of oil around just creates micro-tears that lead to faster scar tissue buildup.

Probably not a good idea for newbs to be using slin pins. Most are only 1/2" at the longest and wont be going deep enough for most spots. Best just to stick with 25g 1" which can be used pretty much everywhere.

I would try to avoid getting gear any where near water. Stick with dry forms of heat to be safer such as hair dryer or probably the best is wrapping up in heating pad.

In the medical field,it has been proven that alcohol pads are not needed and are no longer required for injection sights. Also we no longer aspirate it causes more damage than anything else. Not saying it's right but this is what is being taught, and practiced.
The z-track method is preferred for IM injections.

Makwa's picture

they still always swab me whenever drawing blood or giving injections.

Honeybandit's picture

I still swab also,my patients and myself, too many years of doing it to stop now. But it's not considered best practice anymore same with aspirating.

Makwa's picture

I don't ever recall the nurse aspirating so that is why I never did it either. Tried it once and all it did was wiggle the needle around more causing me needless pip.

Me too. My blood bank actually uses betadine on the site before inserting the needle and alcohol pads on my finger before pricking. I'll stick to the swabs, they're cheap and can't hurt anything. I also shave clean my injection spots, don't want potentially dirty body hair being driven IM.

herpjunkie111's picture

Well don't come crying to me when you drive a dirty butt cheek hair into your gluteus maximus and have to have the left side of your ass amputated, lol.

Good point. You just reminded me of a study I just read on just this topic. I have just post a link to it in a new open forum if you like to check it out.

Its the same as when you visit the doctors or hospital for an injection isn't it - they never swab the site either.

It was inevitable another how to inject your drugs the wrong way was coming.

Someone's been dishing out the karma on it still though...

Water is a source of bacteria so I wouldn't be putting my pin or vial under water. And wash your damn hands before touching anything you want to keep clean. Just thinking alcohol pads kill everything is a no no. Bacteria on your hand grab a swab and rub it on your skin and guess what you just spread some bacteria where you think it's sterile.

It's funny using a Insulin pin for gh every day then you go to pin your test and your 23g looks like the barrel of a shotgun

herpjunkie111's picture

Lol, sure does, though I will say when pinning glutes I can't tell any difference between pinning with a 25 and 23, pain-wise upon injection. The difference for me was I plunged wayyyy too fast with the 23 and got a little pip which I never get with a 25 since the oil moves much slower.

333's picture

I'm going to try 25g next I've always used 23g

herpjunkie111's picture

I'm pinning 1.5ml npp + 1ml test (sust/prop alt) eod so 2.5ml each pin and it takes a minute on glutes with the 25 but haven't gotten the least little bit of pip once.

333's picture

Dam bro you should change your name to hurtjunkie111 for having to pin 2.5 eod lol

herpjunkie111's picture

Lmao, good idea. Trying to figure out how the hell I'm gonna add in my mast prop to tthe mix next week. I may have to start ED pins...

Lol ouch bro

Nice write up. It's always good to put this info out there. I like to use a stainless steel pan for my work station. Theres a reason why doctors use them in a controlled environment and I'm on my bathroom counter. Do the math! Also like to use a blow dryer vs unfiltered running water for warming syringe. Last, I dont massage pre or post pins. I usually pin before a shower and let the hot water massage those areas. + for spreading good info.

Good idea with the dryer for sure

herpjunkie111's picture

Heating pad on high works even better - roll your vial(s) up in it like a burrito and walk away for 10 minutes. Just keep the stoppers pointed up.

Rustys got a pip thread that's pretty detailed.
I do agree with yours though.

x2 This guy isn't a newbie. You shouldn't massage an injection site afterwards either....

Good post for the newbies! Never can be too careful when prepping for an injection!
