Double posted review for source

I was getting a 404 error saying not found when I was trying to post a review for Medicalsupplyliquidators.com. When I clicked go back and tried again I got the same thing. Now when I go to their review page it shows the same review posted twice. Also it keeps saying review is incomplete even though I entered that I've used the product. Any help would be awesome and if a mod can please remove one of the reviews it'd be greatly appreciated.

Your summary doesn't contain how many days it has been used. I only see one review post.
That's the thing, my summary does in fact say how many days it was used... even when I click edit there's a 5 in the box. Also there is only one now cause a mod deleted the other one... Any ideas what could be going on?
If it is just "5", make it "05".
