I just had a 20ml vial of test from another source do that to me. It's not a waste. I just gently crimped it back on with some pliers and tightly duct taped it. No problems at all!
It flew off when I was trying to pop the cap rolled on the counter and hit the floor, no way that's sterile.
You could always sterilize it by submersing it in rubbing alcohol for a minute.
