High SHBG, low test and estro, gyno
Hi all,
My last cycle was in December (tren a and test p).After cycle I continued with TRT 120mg test e per week. In May, I moved to a different state for a job and I had to change insurance and doctor, he was against TRT and we stopped the protocol.
Around June, I started feeling like shit and ran some blood tests and my testo was back to baseline + - 350. Doc asked for a few more months, but I've been experiencing some pain and mild gyno in my right nipple since June . Below are my blood results of this week:
TESTO 372.7 ng/dL 348 - 1197 ng/dL
ESTRADIOL 5.0 pg/mL 7.6 - 42.6 pg/mL
PROLACTIN 12.97 ng/mL 4.0 - 15.2 ng/mL
FSH,SERUM 7.6 mIU/mL 1.5 - 12.4 mIU/mL
LH,SERUM 4.0 mIU/mL 1.7 - 8.6 mIU/mL
SHBG 56.5 nmol/L 14.5 - 48.4 nmol/L
Right asymmetric gynecomastia
I always had low estro, I have never had the need to use a estro blocker, prolactin is normal, besides the low testosterone and I don't see these numbers being all that bad.
Any ideas why the gyno? Is there anything I can do to fix it? Doc asked to wait until 4 more months until take any action.

So, you haven't had exogenous test in your body for 4 months? Are you using any other drugs, currently, besides your blood thinner?
Can you post all the bloodwork from June and your most current one?
Whatever blood thinner you are taking could be the reason your E2 is low.
Hey, First off, get a BMD DEXA test to see if you are not osteoporotic, if you have had such a low E2 for a few years your bones are probably very brittle... It will be a little costly, but better safe than sorry.
Second, my guess is that you have been cutting for too long and knocked you bf% down a lot, which means 2 things. a) adipose tissue is the place where aromatization takes place, which means you don´t convert as much T to E2. b) your cortisol levels are probably sky high and cortisol is well known to increase liver plasma proteins by a lot. SHBG is a liver plasma protein. The only way to get around this is to eat more calories (you don´t necessarily have to gain a lot of weight). Also, cortisol is one of the biggest factors leading to osteoporosis, independently from E2.
The only thing I can´t figure out is why your LH/FSH are rather high and don´t reflect your actual T/E2 numbers. They are released in a pulsatile fashion though, and If your blood test was after a refeed day or something similar the LH/FSH results might be just temporarily elevated.
Try eating like a pig for a month and then retest.
I would imagine your LH and FSH are so high because your e2 is so low. Even though the prolactin is in range that is still high. Your testosterone is not low and don't suspect that as the culprit.
You're not doing anything to manipulate your estrogen? Your cholesterol has to be horrible, especially if that is how your e2 is genetically.
The lipid panel actually was pretty good. I'm not doing anything to manipulate my e2. I believe there is a correlation between e2 and blood cloths, so I'm not sure if my doc sees the low e2 as a benefit.
So, everything was in range on the Lipid panel? Did you get a Thyroid profile, too?

I'm sorry guys, I forgot to mention. I am on blood thinners for life, 13 years now. I'm only 33, but this new doctor seems really worried with recent studies that trt can cause blood cloths. I had two PEs before roids and trt protocol.
I did some research and increasing my SHBG would be my best choice. Free test is lower than the normal range. It could explain my bad hormone ratio.
If i Lower my SHBG and increase my e2, I might not even need to be on TrT.
I have some proviron....I'm not sure if will solve the problem....

I wouldn't go fucking with your hormones. I'd be saying the gyno hurts to any and every medical professional you see about the issue. I'd also go to a different doctor.
Proviron is suppressive. It will only makes things worse I feel.
I'm thinking.out loud here, but after gathering some further history from OP, I would believe we would need a pic with some f/u blood work and maybe a script/drug for something to help with the nipple lump. This scenario sounds more serious in comparison to the other's....

I have a feeling that most docs will be hesitant to put you on trt at 373ng/dl especially with great FSH/LH numbers. Try and address that high SHBG level would be my first course of action.
yeah it can be tricky finding the right doctor and many wont want to touch a patient with high 300's due to concern of liabilities. if he can find the right one though the doctor should identify that a high LH and low test production is a sign his leydig cells aren't functioning very well.
I'd probably take that natural level anyday. My TRT level is only about 200 higher. Curious to what the free test levels are with all that SHBG. Free some more test up and bet the well-being will follow.
I can only speak from my experience but for me, having to low of estro is way worse that having borderline low test. for some reason (I'm assuming extreme stress) my body started going haywire in a similar way to the OP. My estro conversion dropped even though my test was around 350 and my estro ranged around 5-7. It got to the point I could not function, my brain could not process information and as embarrassing as it is to admit, I had ZERO sex drive. it got to the point I couldn't handle it anymore and had to accept the fact that TRT was probably my only choice.
I found a doctor that worked with me and got me on therapy. I'm not fully satisfied with him though at this point and have started to look for a new one. I guess I can just deeply relate to posts like this because ive been there and empathize.
Just out of curiosity if you don't mind me asking, what is your trt protocol currently, makwa?
It has varied over the years. First started off with the gel. Didn't like that. It wasn't working for some reason. Then went to 250mg of test C every 2 wks. Roller coaster was bullshit but there was nothing I could do about it because I had to go to the doc to get shot. Finally convinced him that I am a big boy and can administer my own shots so I went with 125mg/wk. Hated the dam pinning so then I asked for something else and we went with the underarm application. Loved it and currently on it now. Levels around 650 which is perfect for me. Got a letter last week from my insurance company saying they are no longer going to cover topical applications anymore. Bummed out cause there is no way I am going to pay $450/month for my topical prescription. Going to have to find something new now. Doc was mentioning pellets. I don't know if I want to go that route though. Probably going to have to go back to damn pinning again.
Before I moved my doc was discussing the pellets with me, its one application that you get reinserted 3-4 times a year and it is bioidentical not like injections.
You get it "installed" deep in your glute and its supposed to be very uncomfortable for a few weeks and then it acclamates. My doc said it emulates natural production by providing a consistant dose putting you right at age specific levels and a little more. A little costly as even with insurance out of pocket for me for the consult was 300.00 not including the full exams. Most insurance wont cover it because the shit is expensive but it sounds worth it
As no more needles. It would make cycling a little more difficult all things considered but I would stop cycling if I could get natural levels back to normal. Before my drop I was muscular and lean and not even training and then it went to fkn hell.
That's the other thing. I have to check if insurance will cover. My urologist sure seems to be pushing it though.
Its supposed to be a life changer, if you get a chance jump on it. Im going for my consult in 6 months. My doctor was pimping it to me as well so I told him to pay for it.

Get a second opinion from a real Dr. Not a quack in the box. Imo 372 isn't that low but have your Dr get you fixed up stat.

TRT is a lifetime commitment bro. 1st mistake your doc did is to take you off TRT, if he knows any better, he would know that without the exogenous test into your body, you would need to at least be on a SERM or a combo of SERMs to promote your natural HPTA and get your T somewhat optimized that way. 2nd mistake he did is to wash his hands clean and say "ill see you in 4 months". Thats plain BS.
You are feeling like shit as you mentioned, which means its time to go seek another physician. Just so you know MOST physicians wont agree w/ TRT and unless you are having SERIOUS issues they wont recommend any hormones or hormone promoting meds. Your second option is to look for a HRT/TRT clinic. I dont know what state you live in, but TRT/HRT clinics are abundant, but you have to dig and see which ones do it right. I personally have 1 in mind that i use which found to be awesome and affordable. PM if you want me to share that with you, i dont mind.
As far as your lab numbers, they arent so good. I will share with you my opinion bro.
Your Test is Low (border low range), prolactin is high (border high range) it should be at low normal range, your estro is real low which could be causing you joint pain and libido loss, and SHBG is real high above high normal which means ALOT of Test conversion is taking place which means 2 things: Your Free Test is prob low normal, and your Cortisol could be elevated which is a test not even on this panel.
Just sharing my opinion since iv been in this game for TOO long. I can go on recommending supplement and drug regimns to you but that would be counter productive, the best and safest option is to find a good endocrinologist or a good HRT/TRT clinic and let them get a full hormone panel and give you an informed opinion on what the best course(s) of action are.

first off I would be finding a new doctor or at least a second opinion even if you have to pay out of pocket. I've not been through med school so maybe he has reasons I'm unaware of. is he running any tests or scans in the mean time? it just doesn't make sense to wait, feeling like shit and potentially perpetuate the issues.
it is odd that you're having these issues but it's clear something is very off. just something to keep in mind, just because something is in "normal range" doesn't mean it's right. for example, I start getting small lumps as soon as my estrogen gets above 28 or 29 l. it's still qualifies as normal range but clearly it's not normal for me.
why I bring this all up is because your prolactin in at the top of normal.
again, I'm not a doctor so I'm not suggesting to run out and lower you prolactin but a second opinion is what I would seek .
