Sounds like you're talking about the website account creation. They all use a template. There is absolutely no reason to fill out that info except it's required to create an account. Use the following:

your email should be one you can access, e.g., [email protected]

Simple,...... just write in the name you want the source to know you as.

Also, write the name and password down so you don't forget it.

I have a mailing address and and never use my real name. Just playing it safe. I get the mail there regardless of name and if there is ever an issue I would have no idea as to who ordered the stuff!

They ask your name so they know who to ship the pack to I believe. I gave my name signing up. I have had no issues. It’s not like your receiving a lot of stuff.
